Find Balance Health Inc. respects your privacy and handles your personal and health information with the same gentleness we bring to our clinical care. This policy explains our data practices in compliance with HIPAA, CCPA, and applicable state laws.

1. What We Collect

Identity information (name, DOB, ID), contact details, health records (medical history, diagnoses, treatments, lab results, prescriptions), insurance and payment information, and digital data (device info, cookies, app usage). We also collect optional wellness survey responses and patient satisfaction feedback.

2. How We Use It

To provide and coordinate your healthcare, process billing and insurance, send appointment reminders and follow-up care messages, deliver newsletter content (with your consent), improve our services through anonymized analytics, and comply with legal obligations. We never use your data for marketing without explicit opt-in consent.

3. Who We Share With

Only your treating care team, your insurance carrier for claims, HIPAA-compliant cloud and billing partners, public health authorities when legally required, and law enforcement with valid legal process. We never sell your data. Period.

4. Security

End-to-end encryption, SOC 2 Type II certified infrastructure, multi-factor authentication, regular security audits, staff training on data handling, and breach notification within 60 days per HIPAA requirements.

5. Your Rights

Terms & Conditions

These Terms govern your use of Find Balance Health's website, app, and healthcare services. By using our services, you agree to these terms.

1. Services

Find Balance Health provides healthcare coordination, specialist referrals, wellness programs, dental services, aesthetic treatments, weight management, and insurance education. Clinical services are provided by independently licensed practitioners operating within our facilities.

2. Patient Conduct

We ask that you treat all staff and fellow patients with kindness and respect, provide accurate health information, follow treatment plans, and give at least 24 hours' notice for cancellations. We reserve the right to refuse service to individuals who engage in abusive behavior.

3. Payments

Fees are due at time of service unless insurance pre-verification is completed. We accept major cards, HSA/FSA, and offer interest-free payment plans for treatments over $500. Prices may change with 30 days' notice.

4. Cancellations

Free cancellation up to 24 hours before. Late cancellations or no-shows: $25 fee for standard appointments, $75 for specialist appointments. Medical emergencies are exempt with documentation.
